Our services include:

As a safety net clinic, we provide primary health care, chronic disease management and medication assistance to low-income friends and neighbors in Wyandotte County.

As a primary care clinic, we provide routine, preventative care and acute care, such as cold, flu and viruses. We also offer additional services such as:

Chronic Disease Management

(Diabetes, high blood pressure, high cholesterol, depression and anxiety other chronic conditions)

Referrals

Qualified patients may receive referrals for some vision, dental and mental health

Bilingual Diabetes Education

Providing culturally-sensitive diabetes and metabolic syndrome education.

3 Steps to becoming a patient

Step 1

Pick up the patient qualifying forms.

Step 2

Gather the following documents:

  1. Photo ID.
  2. Proof of residency (utility bill or mail with your name, address on it).
  3. Proof of income
    • Paycheck stub (if applicable)
    • Last year’s taxes (if applicable)

Step 3

Start with what you have! Bring in your documents that you can locate. If you’re currently not working or you did not file last year’s taxes, we can help. Bring in what you have, and we’ll help you complete the rest.
